Oasis Senior Advisors (“we”, “our”, or “us”) cares about your privacy. Thank you for taking the time to read our Washington State Consumer Health Data Privacy Policy (“Washington Privacy Notice”). This Washington Privacy Notice covers all personal information processed by our website (https://www.oasissenioradvisors.com) and our services and products (“Services”).

 

  1. Information we collect

“Consumer health data” means personal information that is linked or reasonably linkable to you and that identifies your past, present, or future physical or mental health status. The categories of consumer health data we have collected about Washington residents in the last 12 months are described below:

We will not collect, use, or share additional categories of consumer health data not disclosed in this Washington Privacy Notice without first disclosing the additional categories and obtaining your affirmative consent prior to the collection, use, or sharing of that health data.

Please note that consumer health data protected by this statute does not include data that cannot reasonably be used to infer information about, or otherwise be linked to, you (“deidentified data”). We take reasonable measures to protect deidentified data, and we only process such data in a deidentified fashion without attempting to reidentify such data. We will contractually obligate any recipients of deidentified data to also take reasonable measures and process such data in the same fashion we do.

  1. How we collect your consumer health data (the categories of sources of data collection)

We use different methods and sources to collect information from and about you including through:

    • Direct interactions. You may give us information about you by interacting with our website, by communicating with us via email or contact us page. This includes information you provide by emailing us about our website or Services or leaving feedback on our contact us form.
    • Third-party or publicly available sources. We may receive information about you from third parties.
    • Technical and Communication Data from the following parties. We may collect information from third-parties providers such as X (formerly Twitter), Facebook, and Google Analytics. The information we may collect includes your feedback about our Services on our X account and Facebook page.
  1. Disclosing your personal information

The business and commercial purposes for which we have disclosed your consumer health data in the last 12 months are as follows:

    • Disclosing your consumer health data for business purposes. We have shared the following categories of personal information with our service providers for our business purposes:
      • Category L: Data that identifies you when you’re seeking health care services.

The purposes for sharing such information with service providers include performing transactions, registering accounts, managing our relationship with you, and informing you about our services.

We provide the above categories of personal information for business purposes to the following service providers: our parent company and affiliates, payment processors/service providers, professional service providers such as auditors and lawyers, and consultants.

  1. How to exercise your rights

    1. Confirming our compliance.
      You have the right to confirm whether we are collecting, sharing, or selling your consumer health data and to access such data, including a list of all third parties and affiliates with whom we have shared or sold the consumer health data.
    2. Withdrawing your consent.
      You have the right to withdraw consent for the collection and sharing of your consumer health data. If we seek your consent, we will provide you with a process for withdrawing it.
    3. Deleting your data.
      You have the right to have your consumer health data. If we receive a request from you to delete your consumer health data, we will delete that information from our records, including from all parts of our network, including archived or backup systems, and we will notify all of our affiliates, processors, contractors, and other third parties with whom we have shared your consumer health data of the deletion request. The deletion process may take up to six months after we have authenticated the request.
    4. Request process.
      You can exercise the rights set out in this section above by emailing us at privacy@silverassist.com. Before fulfilling your request, we take steps to verify that you are who you say you are, or you have the authority to act on someone else’s behalf. Therefore, upon receipt of your request, we may request additional information that we need to verify you and, if you are submitting a request on behalf of someone else, to verify that you are permitted to act on that person’s behalf.

      When we contact you to request verification information, please respond and provide the information that we have requested. Depending on the nature of your request, we will verify your identity to either a reasonable or high degree of certainty. This may mean that we need to match two or three pieces of information that we hold about you with information that you provide to us.

      In addition to providing the information we need to verify you or your authority, you must provide us with enough information so that we can understand, evaluate, and respond to your request. We cannot respond to your request or provide you with personal information if we cannot confirm the personal information relates to you.

      If we are unable to authenticate your request to exercise one of the rights listed herein using commercially reasonable efforts, we are not required to comply with the request, or may request that you provide additional information reasonably necessary to authenticate your identity and your request.

      You may request the information set out in this section up to twice in a 12-month period. If your request is unfounded, repetitive, or excessive, we may charge you a reasonable fee to cover the administrative costs of complying with your request or decline the request. Other than where we have noted otherwise, we will respond to your request under this section within 45 days, except that, in some circumstances, we may extend the response period by an additional 45 days if it is reasonably necessary to reply to your request. If we decide to invoke such an extension, we will notify you of our reason for the extension (but we are not required to get your approval for it).

      If we deny one of your requests under this section, you may choose to appeal our decision. You can submit your appeal to us by email at privacy@silverassist.com, stating the basis for the appeal. We will notify you of our decision regarding the appeal within 45 days, including what actions we have or have not taken as a result of it. Our response will state the basis for the decision. If you wish to further appeal our decision, you may contact the Attorney General of the State of Washington by using the resources available here: https://www.atg.wa.gov/file-complaint.

      We will not discriminate against you for exercising any of your rights under this Washington Privacy Notice. You have a right not to receive discriminatory treatment by us for exercising your privacy rights.

Changes to this Washington Privacy Notice will be posted on this site, along with information on any material changes. We reserve the right to update or modify this Washington Privacy Notice at any time and without prior notice.
